Dialectical Behaviour Therapy (DBT)

Introduction

Dialectical Behaviour Therapy (DBT) empowers clients with new skills to cope with painful emotions and reduce the differences in relationships. DBT, in particular, provides four important skills. They are mindfulness, distress tolerance, emotional regulation and interpersonal effectiveness. 

Mindfulness, helps clients to accept and focus on the the present moment without judgement. Distress tolerance, increases the client’s tolerance of negative emotions without making attempts to run from it. Emotional regulation, provides strategies for managing and changing the intense emotions that are causing issues in their lives. Interpersonal effectiveness, equips clients with techniques to communicate with others in an assertive manner retaining their self-respect and helps to strengthen the bond in the relationship.

Applicability

Initially developed to help clients with borderline personality disorder, DBT is now approved by APS for treatment of ADHD, PTSD, Depression, Eating Disorder, Substance Abuse and Borderline Personality Disorder in adults.  DBT skills are helpful for clients who wish to improve their ability to regulate emotions, tolerate distressful thoughts and negative emotions, become more aware of the present and communicate effectively with others. 

Process

Comprehensive DBT therapy is a combination of individual therapy and group therapy sessions. In individual therapy sessions, clients learn the necessary skills from a DBT trained therapist. The therapist keeps the patient motivated to apply the DBT skills in daily life and addresses any obstacles that the client faces during treatment phase. 

The group sessions are aimed to give practice to clients to practice and learn the skills alongside others. Members are also able to share their experience and provide support to each other. 

DBT can be delivered in different ways. Some therapists, prefer only individual sessions while other may only prefer group sessions. There are some who prefer a combination of group and individual therapy sessions.

The Science Behind DBT

DBT was developed by Marsha Lineham, Ph.D. in 1980s to patients with borderline personality disorder (BPD). BPD patients generally experience intense negative emotions when they are interacting with friends, family and partners which they find difficult to manage. As a result such patients have a great amount of differences in their relationships. 

DBT helps them regulate emotions as it is inspired by the perspective of dialectics – balancing opposites. The psychologist works with the client to find ways to hold seemingly opposite views without generating negative emotions, while avoiding the black and white way of thinking. DBT promotes both-or perspective instead of either-or attitude, allowing clients to accept change.
